Who We Are
The Chesterfield County Health Department, along with the Department of Social Services, offers Long Term Services and Supports Screenings (Pre-admission Screenings) which are required for any person requesting care in a Medicaid-certified facility, or for in-home care who is eligible and approved for Medicaid. A detailed assessment is performed by a Public Health Nurse and a Family Service Worker, to determine need and the appropriate setting for care. Based on the assessment and recommendations for care, the individual and their family are given information to contact services available to them.
