CCC+ and Consumer Direction Cardinal Care Medicaid Waiver Services

Family Forward Services offers multiple Cardinal Care Medicaid Waiver programs. Cardinal Care pays for a variety of supports and services, such as Respite Care, Personal Care, Technology, Supportive Employment, Residential Services, and Nursing or behavioral support to eligible adults and children. 

  The EDCD and Technology Assisted waivers were combined into the new Commonwealth Coordinated Care Plus Cardinal Care Waiver (CCC+).  The CCC+ Cardinal Care Waiver is part of a new Cardinal Care Medicaid managed care program, Commonwealth Coordinated Care Plus Health Plan.

This Cardinal Care Medicaid program is for the elderly and individuals with disabilities to receive services in their homes and communities.  These services can be consumer-directed.  The program provides a variety of assistance options to help them, including personal care, fall monitoring or personal emergency response services, and adult day care.
