Cardinal Care Medicaid Waiver Programs, Loudoun

Loudoun ENDependence (LEND) is a satellite site of the ENDependence Center of Northern Virginia (ECNV), serving Loudoun County.  LEND provides Service Facilitation for consumer-directed services under Virginia's Commonwealth Coordinated Care (CCC) Plus Cardinal Care Medicaid Waiver (CCC Plus). CCC Plus provides services that help individuals live in their own homes and community rather than in a nursing home. It is available to individuals aged 65 years or older or to individuals of any age who have a disability. Individuals may be eligible for CCC Plus if they have significant needs for assistance with some daily activities such as bathing, dressing, mobility, meal preparation, light housekeeping, shopping, etc., and have medical or nursing needs.  Individuals who are dependent upon technological support and require substantial, ongoing skilled nursing care may also be eligible.
 
Individuals may receive CCC Plus services while on the waiting list for another Cardinal Care Medicaid Waiver.
