Who We Are

This medical model of Case Management is an Arlington County-funded program that provides Nursing Case Management to older adults and persons with disabilities. An initial home visit is made by the Registered Nurse for a comprehensive assessment of health and wellness needs. The Nurse Case Manager visits monthly and as needed for medical monitoring, health education, coordination of care, and linking with resources.

Home-Based Cluster Care Services: The Cluster Care Program provides eligible adults with long-term maintenance or support services enabling individuals to remain at or return home after hospitalization or skilled facility discharge. Services may include personal care services, nutritional support, environmental maintenance, and support services such as grocery shopping & running errands. Services provided are not medical in nature and therefore are not reimbursed by Medicare, Medicaid, or most health insurance.
