Geriatrics Health Care Center, Veterans

The Geriatrics Health Care Center (GHCC) consists of two services: geriatric outpatient primary care and geriatric evaluation and management, which includes a comprehensive geriatric assessment.

Geriatrician holds certificates in Internal Medicine and Geriatric Medicine from the American Board of Internal Medicine. Services include geriatric assessment, cognitive assessment, ADL, IADL assessment, medication assessment, physical exam, nutritional and diet review, functional assessment, check for sensory impairment, evaluation of falls, osteoporosis evaluation, evaluation and management of incontinence, elder abuse and domestic violence screening, home care services, checks for hearing and vision impairment, depression, memory loss, anxiety and immobility. There is a Nurse Practitioner or Physician Assistant on staff.
