Who We Are

Appalachian Agency for Senior Citizens Care Coordination Department is a resource center for older adults who reside in the four-county service area of Buchanan, Dickenson, Russell and Tazewell counties. The purpose is to empower the client to make more informed decisions that will lead to a better quality of life.

Services include assessment; development of a care plan; assistance with applications for food stamps, fuel/cooling assistance, personal care, and homemaker services, home-delivered meals, transportation, PACE; counseling; coordination of services provided by public agencies and the community. Care coordinators work closely with families and other care providers to assure the provision of services which will allow older adults to remain independent and in their own homes and to thrive in the community.
